For submission of a database listing please read and acknowledge the policy below.
1. Initially:
input shall come from existing databases as created and maintained by organizations or governmental agencies located in Ashtabula County.
said organizations and agencies are founding members of ACOC or recommended by ACOC.
the databases shall be maintained by the creating agency for accuracy, timeliness and correctness through an administrative facility provided by ACOC.
2. Submissions for inclusion in database:
Those seeking to submit a database shall complete an initial application. At minimum, an organization shall provide the following mandatory information and have a purpose consistent with ACOC’s mission and purpose:
Organization Name
Mailing Address (must be permanent organization address within Ashtabula County)
Description of the organization and purpose
Contact person
Phone for organization or contact person (please indicate)
The organization must physically be located within Ashtabula County or specifically serve Ashtabula County residents.
The initial application information shall be verifiable.
ACOC's Board of Trustees maintains the right to reject any submission.
All submissions will be reviewed for inclusion in the ACOC database of Ashtabula County information.
Organizations whose submissions include obscenity, defined as any material that is deemed obscene under the laws of the State of Ohio, and/or libel, hate messages, and violence, defined as any libelous material, hate messages or materials which incite or advocate violence, harm, damage or destruction, shall not be included.
The criteria for inclusion in the database may be changed at the discretion of ACOC.
ACOC reserves the right to remove a database.
3. Policy Statement. Each organization submitting information acknowledges and agrees:
ACOC does not warrant the operation or function of the web site, and will not be liable for any direct, indirect, incidental, special or consequential damage (including lost data, information or profits) sustained or incurred in connection with the submission or inclusion of information on the web site.
ACOC's Board of Trustees reserves the right to change the terms or conditions of the policies of the web site at any time, and any such changes shall be binding on all organizations.
The organizations submitting information to ACOC's web site are at all times, independent contractors, and there is no agency, partnership, joint venture or employment relationship that arises by virtue of inclusion of information on ACOC's site.
Not to use ACOC's web site for any unauthorized, inappropriate or improper purpose.
Acknowledgement
As a representative for the organization seeking to submit information for inclusion within the ACOC informational database, I have read the requirements for submission above and am authorized to accept the terms and conditions expressed on behalf of the organizations.
Appeal Procedure
Applicants not accepted for inclusion in the ACOC database listing or as a link may appeal the decision by completing a "Request for Reconsideration" form and submitting the completed printed form to the ACOC Fiscal Agent at the address below. The "Request for Reconsideration" form may be downloaded below. The request will be reviewed by the ACOC Policy Committee. The Committee will issue a decision within 60 days of receipt of the form. If the request for a database listing or link is rejected by the Policy Committee, an appeal may be made to the ACOC Board of Trustees. An appeal to the Trustees must be made in writing, using the "Request for Reconsideration" form available below. The Trustees will issue a final decision within 60 days of receipt of the form.
